The NHS Trusts below can not locate Grandads medical notes.
University Hospital of North Staffordshire, Health Service Ombudsman : Ongoing
Stoke-On-Trent Partnership Trust, Health Service Ombudsman : Ongoing
Staffordshire County Council complaint
raised by the family and the independent investigator
has produced a 63 page report
this has been upheld on many parts but not in
full, The investigation findings have not been based
on fact. Local Government Ombudsman : Ongoing
The Families ongoing investigation has had a few complications, We made Subject Access Requests to the following : Staffordshire County Council / Stoke On Trent Partnership NHS Trust and the Department of Work and Pensions.
This is how we are getting on lol.
Staffordshire County Council : Acknowledged request then nothing for 2 months and counting.
Stoke-On-Trent Partnership NHS Trust : Acknowledged by telephone and requested evidence of identity
to be returned to them at a wrong address in coventry.
2 months and counting.
Department of Work & Pensions : Acknowledged request by post, requested identity then
Telephoned to advise me that they have no idea how they could
provide the transcript or audio of contact between themselves
and Staffordshire County Council. have heard nothing since.
Now when it comes to the law regarding Subject Access Requests the requested party should respond within 40 days, And should actively assist a person requesting information.
All the above have now been forwarded to the Information Commissioners Office who are dealing with these latest failures.
Raising concerns about possible unfit Social Workers.
I can confirm as stated before a Social Worker responsible for care to my grandfarther has been reported to the hcpc this is a body which is charged with protecting public confidence.
They are gathering information before they make the decision, The family have provided sufficient evidence to assist there decision.
I am hopping that this is the turning point as I have been receiving good feedback from the ombudsman and the ICO.
My own health is still poor and it would be nice to feel this fight for justice is at an end It is only with the help of my Mum and Friends that I have kept the resolve to see this through,The images of my grandads suffering and neglect at the hands of these trusts still haunt me.

How many old folk are being treated like this ?
Hospital Social Workers / Doctors / Nurses who work on these unfit wards with unfit food, Are there eyes shut ?
How long can they stay behind a wall of silence ?
Hopefully with the ombudsman's investigations into these matters the truth can be exposed.
