The Independent Complaints Review Panel Updated 10/10/14

Hi Folks

Further to ongoing events I have decided reluctantly to not be further involved in the trusts process of the Independent Complaints Review Panel.

This is due to the previous months cancellation of review by the trust with very little warning and no communication regarding rechedual.

I have also found out that the trust and the county council have in their possesion fraudulent care notes surounding my late grandads care.

I have asked the trust on this matter however despite contact, There has been no communication.

I can only sumise that the Trust is having to do as it is told by Staffordshire County Council.








Hi Folks


Well this afternoon I went to my very fist Independent Complaints Review Panel at the headquarters of the Staffordshire and Stoke on Trent Partnership NHS Trust.

So I am going to give you an insight into what we did and who was present.

Prior to my attendance I received by post two complaints that had been carried out by the Trust over the past year.

I arrived at the trusts headquarters just before 3pm and signed in and went to a waiting area where others were waiting.

We went into our meeting room and introductions were made within the group, there were representations present from all the disciplines within the trust and myself.

Two of the persons present were the Investigating Officers involved in both the complaints under review.

Each of the two investigations including the findings were placed under very intense scrutiny and the questions being asked of the Investigating Officers were in depth.

Discussions included assessments of Mental Capacity of the elderly and the provision of equipment for nursing staff in the community, further discussions were made into the way information gained from complaints can be validated as lessons learnt.

This was one of the answers I was looking for, It is imperative that the findings from complaints are acted on so that when the trust issue the outcome of complaints, whatever is to be implemented can be checked at a later date.

Unfortunately we live in a world where things do go wrong however there should be no excuse for persistent repeat of the same failings.

It was good to see that both Investigators in these cases had Interviewed the Complainant and every nurse and doctor involved in each case and gained insight into the complaints and had also checked the information they had gained for accuracy.

Both reports had been wrote up in a professional and unbiased manner and if the trust was at fault then full admission of the failure or failings had been disclosed to the complainant.

The meeting lasted for two hours and was not rushed in any way.

I can honestly report that I have gained a new found faith in Independent Investigators after the sad attempt of an investigation that Staffordshire County Council undertook.

In my case the Trust were not responsible for the not so Independent Investigation and today's meeting has only highlighted how bad the County Council undertake Investigations and how if they wanted to could create a process which was unbiased and within the remit of the Complaints Regulations however I think this could be a big ask. 

The professional standard of the Investigation in both cases carried out by the Stoke on Trent Partnership was transparent and based on obtained established facts.

Maybe if the trust had been fully in control of my complaint from day one then maybe many of the fundamental mistakes and bias may not have been made.

This trust does not run away from its mistakes and knows the meaning of Transparency, Accountability and Honesty.
 

Others need to follow by example.
