It is time to place some information into the public domain.
The reason for this blog is to expose failings in clinical care including abuse and neglect and many of the watchers of this Blog have had to wait for a long time for these documents.
After my grandad had his first broken bone in over 90 years he was discharged from the University Hospital Of North Staffordshire to a local satalite hospital under the Stoke on Trent Partnership.
Whilst at this hospital unfortunately I had to raise issues regarding his care and A full internal investigation was undertaken.This was finalized by a final response from the SOTP Trust Chief Executive.
Unhappy with the findings the issue was raised with the Health Ombudsman and the rest is history !!
With reference to my previous Blog "No proof no investigation" I will start today with the list that was taken in to the first meeting with this local hospital.
This list is the base of all concerns raised at that time.
I will provide the outcomes as provided by the Trust over the next few days as lots of scanning required....

Temazipam used when
first at ******** hospital.
Statement that he had reached his potential when walking and
that it would be as good as it could be
.
My grandfather stated that he would call for help and no one
would come.
My grandfather also stated that no one would lower his bed
cot rails when requested.
Apparent failing to provide any Physiotherapy for a period of four weeks
My grandfather stated that all he was eating were chips.
He Gained multiple bed sores whilst at ******** hospital.
He appeared sedated and was not eating and drinking properly.
Sent home with a chest infection, constipation and not
walking.
Sent home 3 hours early after bradwell being informed that
no one would be home until 5pm.
Ambulance staff told to take him home at 1pm in a wheelchair.
Returned home with dirty trousers undone dirty shirt undone
unshaved and very ill.
All discharge paperwork missing.
Only 1 weeks meds and no further information.
Heavily sedated upon return home with high dose of codine
phosphate.
Not eating or drinking.
District nurses not aware of release and neither was GP.
Taken back into ******* hospital 2 weeks later as still
unwell and confused failed discharge the only way they could get him back in was
using his bed sores as an excuse using tissue viability.
Became dehydrated whilst at
********Hospital and then was sent to UHNS very ill confused.
district nurse notes including his DNR did not get sent to
UHNS.
He attended a memory clinic at ******* where
although he was already showing signs of sedation was prescribed further
mitrazapine which has been proven to sedate him.
It is my belief that my grandfather has been sedated out of
convenience and that as prior to his broken hip he was walking had a limited social
life this has been removed from him.
My grandfather has not had the chance to return home a well
man and was unfit for discharge.
