No Proof as No Evidence

I have today heard of yet another elderly person who was living on her own at home and became ill and was addmitted to the University Hospital of North Staffordshire.

She was sent home Dehydrated in a poor state and with no Social Care Folder that had gone with her to the Hospital.

A distressed neighbour was left searching for doctor & social worker and relatives telephone numbers when this poor elderly woman had to be readmitted, She then died.

Yet again this might seem like just notes but the loss of these added to her being released whilst unwell would not have helped.

This form of abuse must stop and the documentation should be treated with more respect.
