Well it might appear that I am a serial complainer but when it comes to elderly care I have been appalled at what could happen in such a short time to one elderly man.
 
My Mum assisted me with the Stoke on Trent Partnership issues and credit to her tenacity in these matters is greatly appreciated.

It was not just one Trust that failed my grandad it was two, One after the other,  Social Workers and Doctors and Nurses worked in these now unused wards and it appears that no one could see what was evident.

These are my own and family's views and the reader should make up there own mind from the documents provided.

Below is My Bullet Points that I wished to be discused when this Trust arranged A complaint meeting.

We never received an explanation of findings, however we did receive a copy of the transcript of the meeting I will post this in the next few days as requires blanks on sensitive data.

Complaint Bullet points

                                     
1.       Food and Fluid / Stool charts Misc Recorded health information inconsistently filled in witnessed by .  **

2.       Loss of fluids and weight whilst in the trusts care.

3.       Time left unchecked on a toilet over 25 minutes witnessed by ** & **

4.       Quality of witnessed food poor standard.

5.       Discharge arranged whilst ****** ******* very ill in the opinions of **\ ** and **

6.       No communication with family regarding DST until after completed and forwarded to **

7.       Failure to correctly establish ******** ******** Capacity after request by the family.

8.       Multiple other Patients in ward requesting attention for toilet and being ignored as witnessed by ** & **

9.       Mr ********** ****** stated that whilst in the trusts care he would call for help and none would come, leading to him attempting to get out of bed over cot rails on multiple occasions and falling in the middle of the ward on multiple occasions.

1.   Over use of medications.

1.   Trust doctor turning away and walking off in the middle of a discussion regarding ********  ********  fluid / food intake.

1.   Unnecessary use of a catheter  

1.   Staff Levels and Ward ** Suitability.

             


With regard to the old fella when I saw him tonight he has an infection and his mood was very low hopefully some chocolate and some prompting will help him feel better tommorow.
