A Letter to the Stoke on Trent Partnership Trust Redacted



Hi Folks as promised A copy of my reply to the local NHS Trust, Unfortunatly at present I cannot provide you all with the letter that prompted this reply.

I will as always let the reader form there own view on this situation.


 
15th December 2014-12-15

Dear Mr Ratcliffe

I refer to you letter dated the 12th December 2014 

Prior to the receipt of Mrs JPs letter of the 15th August 2013 we had had our first meeting with Mr P at that time Mr P and Mrs SH were presented with a copy on DVD of all errors contained within the adult social care file and also the original care notes for the two week period of which I was being accused and the transcript regarding the ambulance call.

The above was all I had been made aware to as the trust refused to release the social care file due to Data Protection

I was advised that all these amendments would be made soon after this meeting.

In transpired after a conversation with the local government ombudsman that they had been provided with an uncorrected earlier document by Staffordshire County Council many months after Mrs JP amendments should have been made.

I further received the full social care file from the LGO which concerned me more by the many entries of fictitious and inaccurately recorded events one of which was a report by myself of care staff bullying my grandfather, This was recorded in the file as myself demanding a new care agency no record was made of the abuse against my grandfather.

With regard to the Financial Audit the monies were not successfully concluded and to date I still state that over a further £1000.00 is unaccounted for when it comes to the day to day financial provision for my late grandfather this has either been stolen by the home or misappropriated by employees in control of his funds.

Neither the trust or the county council or police are bothered by this and repeated requests for a full and through investigation have fell on deaf ears.

I am grateful to your trust for reopening the investigation carried out by the County Council.

On the 4th December we attended the meeting to discuss the trusts findings in relation to the vulnerable adult investigation and the records of investigation contained on the file.

In this meeting I was made aware of the following 

The Failure to provide social care support at a very critical time.

That no records of any of the VA investigations were contained within the file and that no investigation had taken place.

That agencies had not been sharing information.

And the further information regarding the County Councils original investigation.

I appreciate the apology from Mr P and the time taken to meet with both myself and Mr O however It was stated in the meeting of the 4th December 2013 that I would receive a copy of Mr Es findings.

The letter from Mr P does not make any written reference to the failure to provide social care or the specific failings of the agencies involved.

Further the letter makes no reference as to why your trust continually insisted that they held a fraudulent copy of the two weeks home care record which had been utilised for its persistent attack against myself.

I concede that Mrs YS who in my full and frank opinion has committed gross misconduct will not be reported by the trust to the HCPC.

 I concede that Mrs AT Social worker who stated to the Department for Work and Pensions that I was a thief will never be held to account.

I have accepted after receipt of the full social file that it is a poor reflection on professional and unbiased reporting and contains many inaccurate remarks to the point that it would be impossible to ever fully put the record straight as to the many false records are contained within it.

In future there will be no trust in Adult Social care by my family to report anything other than unbiased personal opinions surrounding vulnerable adults.

I concede that I will never see the fraudulent care notes created either for or by the County Council.

In your letter you further make reference that the County Council have formally apologised, to date there is still no existence of any apology other than that made by your Mr P.

I note that you have made reference to how your staff and Mr P have been empathetic to my situation and I cannot disagree the trust have been very understanding.

 My final concern is that I have attempted to raise a complaint with your trust


The issues being the provision of the full findings made by Mr AE as discussed in the meeting on the 4th December 2013.

And the breakdown in communication after this request directly with Mr SP.

To date your Trust refuses to acknowledge this complaint.

I have unfortunately had to escalate this unrelated situation to the PHSO.

For clarity I would like to advise the Trust that my upheld complaint against Staffordshire County Council is a completely separate situation.

In your final paragraph of you letter you refer to the questioning or integrity of your colleagues and officers, I would be grateful if you could clarify where my opinions may be wrong and or provide examples.
With regard to a mutual meeting I will contact Mrs G after Christmas to arrange.
